Patient activation

Patient activation is a defined concept: it is the knowledge, skills and confidence a person has in managing their own health and care.

Activation isn't a fixed personality trait. It varies — someone with high activation around their diabetes may have low activation around their mental health. It also fluctuates day-to-day, situation-to-situation. A bereaved person who normally manages their long-term condition well may suddenly struggle.

Why it matters for SPLWs

The framework expects to:

  1. Know the impact of activation on someone's ability to engage with health and care services and manage their own health.
  2. Assess activation through informal or formal means (some teams use the Patient Activation Measure (); many use a structured conversation).
  3. Adapt practice to support people at different levels — and as the same person's activation moves up and down.

A spectrum, not a binary

A useful working frame:

  • Low activation — feels overwhelmed, may believe their health is mostly outside their control, struggles to ask for help. Needs lots of warmth, small steps, and reliable follow-through.
  • Mid activation — knows roughly what they should do but lacks confidence or the practical means to do it. Benefits from structured planning, scaffolding, and connecting them with peers who have done it.
  • High activation — already self-managing well; values being treated as a partner, not a patient. Benefits from being given information and being trusted to act on it.

Practical adaptations

MidCo-produced action plan with named first step and follow-up; peer leadership programmes; structured tools like motivational interviewing
HighAsk what they have already tried; offer information; agree the outcome you're trying to reach and step back

What to record

In your records, note the person's level of activation as observed, not as a label. "At today's session presented as low activation around money worries; high around physical activity. Adapted plan to focus on one practical action."
